Advance care or advance directive planning is a great way to ensure that you or your loved ones are taken care of the way you want to be in the event of an emergency. Advance directives are legal documents that allow you to specify what medical care or life-sustaining treatments you would like to receive if you become unable to communicate your own wishes. This can include a Directive to Physicians, Family, and Surrogates (also called a Living Will), Medical Power of Attorney and HIPAA Release which allows medical providers to talk to individuals you designate. The documents tell doctors, family members or other people who are close to you the type of help you want when you are sick and how you want to be treated. The documents include instructions on things that you do want and DO NOT want done to you.
